Concierge primary care is a membership-based model that prioritizes access, continuity, and personalization. Instead of billing insurance for each visit, members pay a predictable monthly fee that covers comprehensive primary care services. This allows us to spend more time with you, coordinate your care more effectively, and focus on prevention rather than volume.

Communicate easily via secure messaging, phone, or scheduled visits.

Same or next-day appointments

When something comes up, you’re seen promptly — not weeks later.

Longer, unrushed visits

Appointments are designed to allow time for meaningful conversation & thorough evaluation.

Comprehensive annual exam

A detailed yearly visit focused on prevention, risk assessment, & long-term health planning.

Personalized wellness planning

Care plans tailored to your medical history, lifestyle, & goals.

Chronic disease management

Thoughtful, ongoing care for conditions such as hypertension, diabetes, thyroid disease, & more.

Care coordination

We help navigate referrals, testing, & communication with specialists when needed.

Virtual care options

Secure telehealth visits for convenience when appropriate.

Pricing is age-based and all-inclusive:

  • Ages 18–44: $170/month

  • Ages 45–69: $190/month

  • Ages 70+: $210/month

Multi-member and family discounts available
No copays or surprise fees for included services

Please note: Membership covers primary care services. Labs, imaging, and specialist care are submitted to insurance when applicable.
